Hospital Discharge Volunteer

What You'll Do: 

By providing practical assistance and emotional support, you will play a key role in helping individuals regain independence, prevent readmission, and access community resources. Referring takes just 2 minutes via the CareForMe platform or contact our team.

 

Who would you help?

Pathway 0 and Pathway 1 patients. Patients with low level needs

Patients who live alone or lack immediate support

Patients at risk of readmission

Volunteer training will be provided.

 

You will be trained to:

Help your client settle safely at home (travel with them home from the hospital or meet them there)

Provide practical assistance (e.g. shopping for essentials, medication collection, home safety checks)

Offer emotional support during recovery, or signpost to professional counselling if needed

Connect your client to community resources and activities to prevent readmission and improve independence

Support the client for 28 days

 

Please note you will not be able to provide any personal care or work with patients who have an active Adult Social Care input upon discharge.
